That picture from the sex tape should be someone giving themselves a blowjob. I won't look for it myself, though, because I'm not gay.

Actually the picture I provided is from "Female Trouble". It's a scene where the actor Divine has sex with himself. It's not too far off from the type of humor you are suggesting. However, I think unless you are a film buff you wouldn't know too much about that.
